There is already a thread with all the information you need. If you have further questions feel free to ask here... One small addition though, most of the people had only luck to flash the card using ATIFlash v3.25. I'd try with that version and only if that doesn't work, I'd use the highest version available of the DOS flash tool. Don't ever use ATIWinFlash. How you boot into DOS mode it's totally up to you. Because you don't have a floppy drive and you're not able to boot via USB, you should create a bootable CD. 
To be sure put every version of ATIFlash from version 3.25 up to the newest available and also the BIOS file. I've booted with the Windows Millennium floppy, but you can also use Windows 98 boot files to create the bootable CD. Much easier would be to use an old hard disk, format it and make it bootable and use Windows 98 files to boot into DOS. Then you are also able to make a backup of your current BIOS which you can't with a CD.
